Here at Lewisham and Greenwich Trust, we strive to deliver the very best care to our elderly population and we have recently established a Frailty Pathway.
The Frailty Pathway will deliver acute intervention to ensure that the patient’s length of stay is reduced in hospital, and then through working collaboratively with community services the Frailty Pathway will support safe and effective discharge.
Currently there are opportunities for Band 5 nurses to join our teams that deliver care to frailty patients. Join our team and you will enable positive outcomes for our elderly patients, you will have experience of working in an acute environment, combined with an interest in care that encompasses rehabilitating patients to discharge.
Our ward teams will develop your expertise through the variety and complex needs of our patients and through the Frailty Pathway you will provide patient centered care improving outcomes and reducing re admissions. You will be welcomed into a team that achieves excellent care through close working with both the patient and their family to deliver truly focused care.
You will be supported by a band 7 Ward Manager whose role is to support staff education and development, creating a culture of learning within these areas and the wider organisation. Matron Eliza Mathew is looking for innovative individuals that would look to influence the care of patients on Hawthorn Ward.
